How to create staging test account?

Do note that: 

1.  Please note that you can only create a maximum of 5 test accounts.
2. Test accounts usage is not restricted to a specific app.
3. Test accounts created in SDP is only for Login testing. For Myinfo testing, please use the following
   1. Myinfo v3v4: Mockpass
   2. Myinfo v5: Test Personas
